The Lorain County Board of Elections currently sends out letters to each poll worker to tell them their assignments (job code along with their precinct and polling location they will be working) and two class choices (the class choice is based on the precinct they will be working at). The poll worker then returns a portion of the letter stating whether or not they will work that election along with the class they want to attend. We would like to automate this process. The requirements are as follows: • Allow us to send out e-mails to our poll workers. The e-mail will contain a few paragraphs along with their County ID, Name, Job Code, Precinct, Polling Location and the Polling Location Address. And a link to the form in Zoho. We will need to track undeliverable e-mails. • The form will allow the poll worker to enter their County ID. Once entered, the form will populate with their First Name, Job Code, Precinct and Polling Location (the First Name, Job Code, Precinct and Polling Location will not editable). The poll worker will have a choice of the two classes (drop down box – based on the Precinct) along with two radio buttons. The Radio Buttons will be “UNABLE TO WORK THIS ELECTION” and “CONTACT ME NEXT ELECTION”. • Have an e-mail automatically generated to send to the poll worker confirming class assignment once the form is completed. This will only be sent if a class assignment is selected. • A report (both CSV and Written) needs to be generated for each response. The report will need: County ID, Name (first & Last), Precinct, Class Chosen and the values of the two radio buttons.
