Pop-Up Signup forms

I would like to place different pop-up signup forms on different pages of my website

I would also like the form to be chosen by the customer's behaviour (eg if they have previously visited the page or site before.  Is this possible?  I also want to be able to design the form background image, copy etc.  Can this be done?
