Populating form fields with webpage details

Hi all

I have a lead form created using Zoho Forms. The form is intended to exist on many different webpages.

I would like the form to pull in the page meta title and url and pass them to hidden form fields. How do I go about doing this?

I'd also like to pull in the Google Analytics Client ID, if possible.
