I have a customer satisfaction survey that includes a question about being contacted for help with an issue. If they answer yes, they enter in a name and phone number as well as an issue.

So, if they answer yes, I want to write the customer's name, phone and issue into a spreadsheet (and ONLY that information, no other questions) so that my client can act ASAP.

Is this possible, and if yes is there a For Dummies guide as to how to do it?
