Greetings

i was wondering if there was a way to create a customized popup/modal and be able to show it on a form using some sort of button triggers or so. 

i'm having a form where i want to include explanations and instructions on how to fill it and i want to include these instructions in a modal that could be triggered by something like a "help" button/icon

is such a thing possible?
