look in my business my agents calls the customers and record  the survey . 

I uploaded the leads in the CRM integrated the CRM with survey and got the survey questions . But in the survey form My requirement is that whenever the user enters the contact number he should get he details of the lead Or from the lead itself we go and initiate the survey so that we have the survey details . How to do that??
