Recently, we've seen how survey end pages can increase your survey’s response rate. In the same way, survey introduction pages play a huge role in carrying out your survey’s purpose. There are three parts that make up a great survey introduction page: 

Title - Give your survey a title that conveys its purpose to the user and at the same time grabs their attention. The introduction title should be constructed based on the questions used in the survey and your brand.
Description - The survey introduction description should contain complete information about what the survey will ask the user, how the answers will benefit the survey creator, and where the survey creator will use the survey data provided by the user.
Begin survey button label - This is the call-to-action button that begins the survey. Give the button an interesting name that will motivate the user to respond to your survey.
