Yes, you can include contact's custom fields in your signup forms but this is only applicable for Long forms and not for any other type of forms. To include custom fields in a long form:
Navigate to Audience -> Forms.
Click the Create New button at the top right.
Specify a name for the form at the top, choose a topic and a list.
In the right side, click the Long form. Hover over a template and click Choose.
In the template editor's left side, under the Customize Fields section you can drag the existing custom fields into the form's body.
To create a new field, click the + icon next to Customize Fields, and add your field, and save it. Then, drag that field into the form's body.
Once all changes are done, click Save and Close at the top right.
No. By default, the Contact Email will be mandatory. But, you cannot mark other fields as mandatory in a signup form.

It is not possible to restrict signup forms to work only on specific domains.
If the same contact submits a form more than once, then:
the same contact won't be added the second time
but when submitting the second time, if there's any change in the data (such as change in the name) then that will get updated in the existing contact

No. Forms are webpage, popup, or embedded widget that lives on your website, and the available form codes (IFrame, Website, No CSS, No Script, Raw HTML, CMS tools, Form URL and QR Code) are all meant for embedding on a website, not inside an email.

If you have enabled double opt-in, then your contacts will get added to your Zoho Campaigns organization only after they confirm the double opt-in. Therefore, only after they get added to your organization, the response actions you've set up will trigger.

Here are the complete steps:
Navigate to Automation -> Workflows.
Click Create New and then Start from Scratch at the top-right corner.
Edit the name of the workflow and include a description at the top and click Update.
Under the Trigger section in the left, drag the Form Submission component and drop into the canvas. Click the Select Form option inside this component, choose Any from, and click Save.
Under the Action section in the left, drag the Send Internal Email component into the canvas. Click the Configure internal email option inside this component, provide the Subject, choose who should receive this email, specify the content of the email and include merge tags by clicking the @ symbol and click Save.
Under the Action section in the left, drag the Remove From Workflow component into the canvas.
Connect all three components and click Launch at the top right.
