I am trying to create a more user accessible database for auditing best practices for an insurance claims department. There would need to be ability to upload an excel sheet of basic header information then have an input area for audit and review. I see this as a two sectioned form - header information from input at top and audit section on bottom. Each audit criteria would have a drop down to choose from and then notes (required if one user selects a specific audit finding i.e. does not meet expectations). This is not a financial audit.
