I am looking for assistance in creating a db that will allow for 3 primary components: 1) a new user to registration with password creation and basic questions 2) Once a client is registered, we need the ability for each client to record incidents through our website. Users may log many incidents which will include several free text, drop downs and file upload fields 3) User can login and see all incidents they have entered It is important that each user is restricted to their own incidents. Forms will need if/then features that will expand form if certain answers are selected. Budget is tight as we are a start-up business but hopefully will lead to more work in the future as the business model is refined. More details will be available in the coming weeks. Thanks, Bill
