Form Deactivated

Good morning,

I have created the form several times. but every time I create a form, my form is always deactivated. I have read in the community that the forms are deactivated because the form contains password and CVV information. in fact my form only contains name, signature, and class for my activity attendance. I really need this form urgently. Please Help.
