I would like some help creating an application that merges information from a combination of selections (ie, checkbox, drop-down, radio button, text entry) with pre-formatted forms. The intended result is to allow a user to 1) select a form; 2)enter the relevant information; 3) allow online submission, printing, or emailing of the completed form. There are only about 10 forms, most are single-page and with just a few fields. Some of the data for the forms will need to come from a table that must be created containing the information to be used in drop-downs, radio buttons, etc. This forms application will ultimately reside on a Wordpress blog site.
