Mentor booking

Mentor booking

I would like help constructing a booking form with integrated text alerts. I have mentors arriving in the school, I want a booking form to be activated when they sign in using their phones, students to be alerted of their arrival by text, the students then being able to book timeslots with the mentor. I would also like the student to be alerted as to when their slot is via text as well. If possible, I would like the sign in to be location based, using something like Foursquare.
