An online registration form for multiple and ongoing on-site corporate blood draws. Eligible participants at each corporate client would be uploaded with basic info, such as name, phone, email, and worksite location, and only these people would be able to sign-up. Plus, we will want to track non-participants for future follow-up communication and reporting.
There are four basic screening options that an employee would select from a drop-down menu, which would require "smart-logic" asking one more question. For example, if "Mail me a screening test kit" was selected, we would ask, "What address do you want your screening test kit mailed to?" If they selected, "Attend an onsite screening event," then a link would be provided to our online appointment tool, so they could schedule their session. A login for the client would be needed to see real-time reports/dashboard based on those who signed up.
A pie graph that would reflect total eligible participants, then a breakdown of the 4 options employees have selected, plus those who have not selected an option, so we can reach out to them to sign-up. If there were multiple worksites for one client, then a breakdown of participants per location would be needed, compared to those who had not signed-up yet. Lastly, automated and scheduled email reminder notices would be great to have based on the option they have selected.
