You can easily configure a form to dynamically navigate patients to different sections based on their input regarding the type of appointment and pain severity level.
Let’s walk through how Zoho Forms can help streamline your dental appointments and prioritize emergency cases with ease.
How it works: Steps to set up conditional navigation
1. Create the Dental appointment booking form
You’ll start by building a Dental appointment booking form. Add a Dropdown or Radio Button Button field where users will select the Pain Level (Mild, Moderate, or Severe). Add any other necessary fields for the appointment booking process (personal details, address, etc.).
2. Set up Page Rules to display relevant pages based on user input
You can use one of the types of Conditional rules: Page Rules available in Zoho Forms to skip to the relevant page based on your patient's condition. In this case: the form will respond differently depending on the pain severity chosen:

If the user selects "Severe", they are directed to an Emergency Treatment Details page to provide more specific information for an immediate appointment to be booked.
- If the user selects "Mild", the form simply proceeds to a General Appointment Summary page.
This functionality streamlines the user experience by tailoring the form flow according to the patient's condition severity to ensure that relevant data is collected efficiently.
To achieve this:
- Click Rules in your form builder and navigate to Page Rules.
- Click Configure Now.
Select the page for which you want to configure rules. In this case, we will select Page 1: Patient Information.
- Click on the + Rule button
- Specify the conditions for Rule 1:
- For this use case, to configure the IF condition, you will need to select the radio field named "Pain level" then select "Is" followed by "Severe" in the dropdown menu.
- Next, you must select the page to be skipped to, when this condition is satisfied: Page 3: Emergency Treatment Details.
- Likewise, specify the conditions for Rule 2 by simply clicking the + in the bottom right hand corner and repeating the steps with a few changes:
- Select the "Pain level" then select "Is" followed by "Mild" in the dropdown menu.
Select the page to be skipped to, when this condition is satisfied: Page 3: General Appointment Summary.
You must also select which page to skip to when none of the above rules are satisfied.
- Click Add.
- To configure more rules to assign tasks, click +Rule and repeat the above steps.
3. Test your setup
Once you’ve set up your page rules, we always recommend testing your form! Fill in a few test applications with different pain levels checked and ensure the form is directing respondents to the appropriate pages.
