Tracking Referrals
To track custom referrals, you need to add a tracking parameter at the end of your Form Permalink (URL) before embedding the iframe or JavaScript code in your webpage.

This will help you know if a customer filled out your form from your Home page or Contact Us page.
Tracking referrer name when you have embedded form using JS Code

In your form builder, navigate to the
Share
tab and click
Embed in the left menu.

Click Javascript
and click Customize Now.

Select Include URL Parameters and set Referrer Name as the Parameter Name and enter the Parameter Value as shown.
This helps you track the source from which the form was filled.
